Use this form if you are doing appointment reviews over Zoom (or similar). Upon completion (and pressing 'Submit'), the form will be sent to the person being reviewed and copy to the reviewer.
The person being reviewed will need to reply to the email (with the completed form), either accepting or rejecting it. If accepted, the form can then be forwarded to the Appointments Secretary to allow Compass to be updated.
